Applications are invited from intending and eligible candidates, who are citizens of India and having Registration Number in Employment Exchange in the State of Assam as defined in the Articles 5 to 8 of the Constitution of India in the prescribed Standard Form of Application as given in Part-IX of the Assam Gazette, for filling up the following vacancies under the establishment of the Directorate of Health Services, Assam, Guwahati -36, for 329 nos of Grade-IV posts in 47 Nos. of newly created Model Hospitals of the District level in the State, and carrying the following Post and Pay Band plus Grade Pay with other allowances as admissible under rule.

Educational Qualification:
The Candidate should be VIII passed.
Scale of Pay:
Pay Band -1, Rs.12,000-Rs.37,500/- + Rs.3,900 Grade Pay
2. Name of the Post: Ward Boy
No. of Posts: 141
Educational Qualification:
The Candidate should be VIII passed.
Scale of Pay: Pay Band -1, Rs.12,000-Rs.37,500/- + Rs.3,900 Grade Pay
3. Name of the Post: Ward Girl
No. of Posts: 141
Educational Qualification:
The Candidate should be VIII passed.
Scale of Pay: Pay Band -1, Rs.12,000-Rs.37,500/- + Rs.3,900 Grade Pay
Reservation:
30% reservation of vacancies of post as per Women Reservation Act, 2005 and 3% reservation of vacancies for physically disabled persons will be followed as per Rule.
Age Limit :
Candidate must not be less than 18 years and more than 43 years as on 1st January 2017. The upper age limit is relaxable by 5 years for SC/ST candidates.
Application Fee:
1. For General candidates= Rs. 250/-(Rupees Two Hundred Fifty) Only.
2. For SC/ST/OBC candidates= Rs. 150/-(Rupees One Hundred Fifty) Only.
3. Candidates having BPL certificate= NIL. Fees should be deposited only through Treasury Challan in the Head of Account “0210-M&PH-01-Urban Health Services-800-Other Receipt”.
How to Apply:
The applicants should submit their application along with photocopies of all testimonials (self-attested) regarding qualification, age, caste, employment exchange registration card, Caste certificate (in case of reserved category candidates), Disability Certificate (in case of Person with Disabilities) issued by competent authority, Original Treasury Challan, a copy of BPL certificate in case of candidates of BPL families and two recent passport size photograph of the candidate with his /her signature at the top. The self-addressed envelope of 10’x4.5′ affixing postal stamp of Rs.5/- must be enclosed with the application.
Applications should reach to the respective Jt. Director of Health Services of the District on or before 03.01.2018.
Any application received after the closing date as well as an incomplete application shall not be entertained and summarily be rejected.
Detailed list of eligible candidates for each post along with the details of selection procedures shall be notified in due course of time and shall be displayed on the website (www.dhs.assam.gov.in)
